Please Note: Inline with C.A.P.S. insurance all members must be over 4 years old in order to participate in C.A.P.S. Clubs / Camps / Activities / Events.

Annual Registration Fee: €30 per family

Proof of Diagnosis

Acceptable documents include: AsIAm Card, front page or letter from a clinician confirming the diagnosis of autism this is inline with our insurance so unfortunately the registration process can not be completed without it.

Forms may be returned by email or post and the annual registration fee (€30 per family) may be made via bank transfer or alternatively in person. Please contact Trina via email: admin@capssupport.ie or phone on 0863540899 for further details/assistance.
